LIFELINE PROGRAM TERMS AND CONDITIONS

Boycom Cablevision, Inc. (“Boycom,” “Company,” “we,” “us,” or “our”)

Effective Date: January 1, 2026

Service Area: Missouri


1. Overview of the Lifeline Program
Lifeline is a federal assistance program administered by the Federal Communications Commission (FCC) that provides a monthly discount on qualifying telecommunications and broadband services to eligible low-income consumers. Boycom Cablevision, Inc. participates in the Lifeline program in compliance with FCC rules, Universal Service Administrative Company (USAC) requirements, and applicable Missouri regulations.
2. Eligibility Requirements
A customer may qualify for Lifeline service by meeting one of the following criteria:
2.1 Income-Based Eligibility
Household income at or below 135% of the Federal Poverty Guidelines.
2.2 Program-Based Eligibility
Participation in at least one qualifying government assistance program, including but not limited to:
• Medicaid
• Supplemental Nutrition Assistance Program (SNAP)
• Supplemental Security Income (SSI)
• Federal Public Housing Assistance (FPHA)
• Veterans Pension or Survivors Benefit
Eligibility must be verified through the National Verifier or approved documentation.
3. One-Per-Household Limitation
• Only one Lifeline benefit is permitted per household.
• A household consists of individuals living at the same address who share income and expenses.
• Lifeline benefits are non-transferable and may only be received from one Lifeline provider at a time.
4. Lifeline-Supported Services
Boycom offers Lifeline discounts on broadband, voice, and bundled services, subject to service availability and FCC program rules.
• Standard Lifeline discount: $9.25 per month
• The discount will not exceed the total monthly charge for qualifying service.
5. Current Lifeline-Discounted Service Plans (Missouri)
The following Lifeline-eligible service plans are currently offered by Boycom in Missouri.
All prices shown reflect the $9.25 Lifeline discount applied.

Important Notes:
• Availability of fiber services depends on location.
• Taxes, surcharges, installation fees, equipment charges, and optional services may apply
and may not be fully covered by Lifeline.
• Prices and plans are subject to change in accordance with FCC rules.
6. Customer Responsibilities
By enrolling in Lifeline service, customers agree to:
• Provide accurate and complete information.
• Notify Boycom or the National Verifier within 30 days if:

o Eligibility status changes
o Household composition changes
o Another household member receives Lifeline benefits

• Use the Lifeline-supported service at least once every 30 days.
• Complete annual recertification when required.
7. Recertification and Reverification
• Lifeline customers must recertify eligibility annually.
• Failure to recertify within required timeframes will result in de-enrollment.
• Boycom is not responsible for loss of Lifeline benefits due to failure to respond to
recertification requests.
8. Non-Usage Policy
• Lifeline service is subject to termination after 30 consecutive days of non-usage.
• Usage includes making or receiving a call, sending a text, or using broadband data.
• Required notices will be provided prior to de-enrollment.
9. Fraud, Misrepresentation, and Penalties
Providing false or misleading information may result in:
• Immediate termination of Lifeline service
• Potential repayment obligations
• Penalties under federal law
Customers certify under penalty of perjury that all information provided is true and correct.
10. Service Changes and Limitations
• Lifeline discounts apply only to eligible services.
• Lifeline does not guarantee free service or uninterrupted eligibility.
• Service offerings, features, and pricing may change due to regulatory or program
requirements.
11. Transfer of Lifeline Benefit
• Customers may transfer their Lifeline benefit to another provider in accordance with FCC
rules.
• Transfers are generally limited to once every 60 days, unless an exception applies.
12. Privacy and Use of Customer Information
Customer information is collected, used, and shared in accordance with:
• FCC Lifeline regulations
• USAC requirements
• Boycom’s Privacy Policy
Information may be disclosed to regulatory agencies or program administrators as required by
law.
13. Termination of Lifeline Service
Boycom may suspend or terminate Lifeline service if:
• The customer no longer qualifies
• Required documentation is not provided
• Annual recertification is not completed
• Fraud or abuse is suspected
• Required by FCC, USAC, or Missouri regulatory authorities
Termination does not relieve customers of responsibility for non-Lifeline charges.
14. Governing Law
These Terms and Conditions are governed by federal law and the laws of the State of Missouri,
without regard to conflict-of-law principles.

16. Customer Acknowledgment
By enrolling in Lifeline service, the customer acknowledges that they have read, understand,
and agree to these Terms and Conditions and certify that all information provided is accurate
and complete.
17. Missouri-Specific Lifeline Disclosures
(Missouri Public Service Commission – PSC-Style Language)
17.1 Availability of Lifeline Service
Lifeline service is available only to eligible Missouri residents and only in areas where Boycom
provides service. Participation in the Lifeline program does not guarantee availability of all
service plans at all locations.
17.2 Non-Discrimination
Boycom provides Lifeline service on a non-discriminatory basis. Lifeline customers are not
denied service or treated differently solely due to participation in the Lifeline program.
17.3 Deposits and Credit Requirements
Lifeline customers are subject to the same credit requirements, deposits, and service conditions
as non-Lifeline customers, except where prohibited by law. Boycom Service Terms & Conditions

17.4 Billing, Taxes, and Fees
Customers remain responsible for all applicable taxes, fees, surcharges, installation charges,
equipment fees, and optional services not covered by the Lifeline discount.
17.5 Suspension and Termination
Boycom may suspend or terminate Lifeline service for non-compliance with program rules, nonusage,
fraud, or as required by FCC, USAC, or Missouri authorities.

17.7 Changes to Lifeline Program Rules
Lifeline services are subject to modification or discontinuation based on federal or state
regulatory action. Customers will be notified of material changes as required by law.
17.8 No Guarantee of Program Funding
The Lifeline program is funded through the federal Universal Service Fund. Continued
availability of Lifeline discounts is not guaranteed.
17.9 Missouri Regulatory Authority
Nothing in these Terms and Conditions limits the authority of the Missouri Public Service
Commission or the Federal Communications Commission.
